the best questionnaire questions and examples

To help you build effective forms, we've collected this list of questionnaire ideas and examples. These ideas cover everything from HR and marketing strategy to customer support and events.

A questionnaire is more than just a list of questions, its a strategic tool designed to extract specific, actionable information. Unlike creating a quick poll, a questionnaire is often structured to gather comprehensive data on a subject—whether it's a new client's needs, an employee's performance or a patient's medical history.

But a questionnaire is only as good as its graphic design. If it's too long, confusing or irrelevant, your completion rates will plummet.

Questionnaire ideas and examples FAQ

How long should a questionnaire be?

Unlike a poll or survey, a questionnaire cane be longer. However its still important to respect your users time and effort and not over extend it. The following can be used as a good rule of thumb:

  • Transactional (Feedback): 3-5 questions.

  • Tactical (Onboarding): 10-15 questions.

  • Strategic (Market research): 20+ questions (incentive recommended).

How do I avoid bias in a questionnaire?

Its important to avoid leading questions that push respondents towards specific answers. For example a leading question would be, How much did you enjoy our amazing service, whereas a non biased one would be, How would you rate the service you received?

What about privacy (PII)?

If your questionnaire collects Personally Identifiable Information (Names, Emails, Health Data), you must include a privacy policy link and a consent checkbox in the US. Always check your platform is GDPR/CCPA compliant if you're collecting data across borders, in Europe, for example. It's your responsibility to make sure that your questionnaire doesn't breach the law where you plan to run it.
