Paid Membership Form for NonProfit

I am creating a website for a nonprofit. I am seeking a solution to have potential members fill out a form and pay their membership online. Our membership renews each year in July regardless of when the member joins, so a 6/12/monthly renewal doesn’t work for us. I’ve experimented with the Paid Plan option, but don’t understand how to require the person to fill out a form before/after purchasing the membership. Thank you for your assistance!
