Create a Custom Form & Connect It to a Database

I am trying to Create a Custom Form & Connect It to a Database, when i add my dataset and choose “Read & Write” as the dataset mode. The form doesn’t work. WHY is that?

May be you have required fields, check it
Also open your console, you can open it from the keyboard: press Ctrl+Shift+J
Is there any errors?
