Auto Fill Form by filling just ID

Hi, i would like to actually build a form where it can be auto fill.
User journey would be:

  1. User filled in their ID
  2. Lead to form page and data will be autofilled based on the existing data base.
  3. Submit, information will be submitted to the data base.

Can anybody help on this?
