On a few browsers the below code is showing underneath ‘when should we contact you?’

This doesn’t show on safari at all… I can’t figure out how to get rid of it!

Any suggestions?

Weird. I loaded your page using Google Chrome and Internet Explorer and didn’t see this issue.
