Processing Your Claim
Processing your claim is easy. There are just 2 simple steps.
1)Downloadthe appropriate claim form (for Allstate Claims the fax number is on the form)
2) Attach a copy of the EOB, Explanation of Benefits that you receive from the hospital or doctors office and fax along with the claim form.
Note: Most claims are turned around in 5 days if you have filled out the form correctly.
