At 18 he's trying to be independant and as a newly (1yr ) diagnosed type I diabetic I was trying hard to back off and give him some freedom. I discovered that he was far too high for nearly 3mths, and he hadn't done much to change the fact. His 3mth Ave was just over 14. I have increased his night Lantus dose initially by 2 units and then a further 2 after a week. His dose is 24 units. His levels r much better. His Physician has told him not to worry too much with the diet, but to control his levels with his insulin. We have removed most of the sugar from his diet and are using the Sustained eating recipe books. I have to admit that we still eat too many carbo loaded foods of bread rice pasta. We use brown rice and low GI Sasko brown bread and we add oat bran to sauces for thickening. We are trying to increase vegie and fruit intake. Not always successful. We generally have a cooked meal for supper with meat, rice or potato and at least 2 to 3 vegie types. Gravy is always a must - so we do do saucy foods. We have reduced the fat intake, but Andy is very thin and tall with a cholestrol level of 2.8 We really battle with ideas for snacks when his sugar level is high but he feels really hungry. Esp late at night when nuts and fruit aren't what he wants. Any ideas? Dear Sister Anita Firstly, THANK YOU for not giving up on me. NON-ADHERENCE VS ADHERENCENon-adherence is a major problem within healthcare today. Non-adherence to prescribed medications causes nearly 125,000 deaths annually. Some figures suggest that greater than 60% of the population do not get their prescriptions filled and of those that do, another 50% do not take the medication properly, or at all.¹Non-adherence can constitute many forms: not having prescriptions filled, taking incorrect dose at incorrect times, forgetting doses or stopping before the recommended time.The alarming rate of non-adherence includes patients taking drugs for serious conditions such as high blood pressure, high cholesterol and diabetes. Key approaches in managing health care through Health WindowChronic diseases are those that can only be controlled and not, at present, cured.They include diabetes, asthma, arthritis, heart failure, COPD, dementia and a range of disabling neurological conditions.Living with a chronic disease has a significant impact on a persons quality of life and on their family. The incidence of such diseases increases with age.Many older people live with more than one chronic condition and this means both the patient and the family faces challenges which include medical, social and psychological challenges. The key approaches in managing chronic disease through Health Window related to:•Self-care and self management supports people to take on an active role in managing their own care. It is to help people to manage their specific conditions, and to adopt approaches that prevent these conditions from getting worse and reduce the risk of further conditions.¹•Knowledge management is the ability to identify at-risk groups within the population, carry out needs assessments, understand resource and activity levels and identify trends.¹The complexity and challenge of managing chronic care increases as people develop multiple conditions as they age. However, while individual people may have complex conditions, the system for managing chronic care can be simple. Different interventions can be used for patients with different degrees of complexity.¹Level 1:With the right support many people can learn to be active participants in their own care, living with and managing their conditions. This can help them to prevent complications, slow down deterioration, and avoid getting further conditions. The majority of people with chronic conditions fall into this category so even small improvements can have a huge impact.¹Level 2:Disease/care management, in which multidisciplinary teams provide high quality evidence based care to patients, is appropriate for the majority of people at this level.¹ This means by managing the condition proactively, following a structured process in managing these conditions or illnesses.
