You have a choice where you want to go for treatment. Choose Bloom & Associates because we are small enough to care about you personally, we offer great value, and work hard to make your time with us special.

We have offices in Topeka, Lawrence, and Eudora, Kansas. Our staff would be happy to answer questions Monday through Friday 8 AM until 5 PM. 785-273-7700

If you do not see your insurance listed to the right, please contact our billing office and they will be glad to assist you with any questions you may have. We will contact your insurance for you to see if you are allowed to receive your treatment with us and help get it pre-certified if your plan requires your treatments to be pre-certified.

We will complete appropriate insurance forms for all insurances. Ultimately it is your responsibility for payment of your account. The agreement for payment is between you and your insurance company, not us and your insurance company. Verification of benefits does not guarantee payment by your insurance.

Payment plans are available on an individual basis.
