Please find attached the permission slip for flu shots. Download, fill out, and sign for EACH player. The players will need to bring in to eceive a shot. Flu shots will probably be given the last week of September after practice. I will inform everyone of the date (dates?) when finalized.
Please send me an e-mail to footballflushot_gmail.com so we have an accurate count and will have enough doses available.
