As an ATU 615 member you can be reimbursed for eligible medical expenses and premiums paid if you make a claim through your Health Spending Account. Attached please find a brochure on the ATU 615 Health Spending Account and a copy of the claim form.
Please note, there is a claims limitation period of 30 days allowed at the end of each policy year. During this period claims may be submitted for allowable expenses incurred in the previous policy year and payment will be made using any balance you may have from the previous policy year. Submission of claims after the claims limitation period will not be eligible for reimbursement. Therefore, if you would like to be reimbursed for eligible medical expenses which you or a dependent incurred in 2010, you will need to submit your claim for reimbursement ASAP (deadline is January 30, 2011).
In addition, For a quick and easy way to access your personal claim information with Saskatchewan Blue Cross, you can register under the cardholder website.
Registration will get you access to:
· detailed information on your health and dental coverage
· ability to view and print claims history and/or payment history
· print claim forms anytime
· access to information on requesting additional cards
· access to change your personal information such as address, phone number and e-mail address
· inquiry into whether a specific drug or certain dental procedure is covered under your plan
